Warns when a document doesn't include a DOCTYPE. Ignored for document fragments.

An included DOCTYPE that uses an obsolete public/system identifier form is no-obsolete-doctype's concern, not this rule's — any DOCTYPE, obsolete or not, satisfies this rule.
